COMPENSATION AND BENEFITS Family Medicine (FM)

• Salary Year 1 "including"  $150K sign on bonus = $450K First Year
• Base salary + Production incentive = 45% of cross charges over $415K
• Call paid $75.00/hr. Weekday, $110.00/hr. Weekend
• Sign on Bonus: $150K
• CME - $5K
• Retention bonus - 5% of annual salary
• Vacation - 6 weeks annually
• Holidays - 6 days annually
• Relocation - Paid
• Health, Dental, Life, Long-Term Disability, FICA - SS and Medicare
• 403 (b), Retirement plan 5% of salary
• Malpractice and Tail

RESPONSIBILITIES AND FACILITY DETAILS Family Medicine (FM)

• Short Term Acute Care
• Clinic 8am-4pm M-F
• Call rotation 1:5 - IP/ER
• NO OB required “but” is an option if interested
• Skilled Nursing Facility/Swing beds

QUALIFICATIONS AND SKILLS Family Medicine (FM)

• BC or BE within 1 year of employment
